From 2018 to 2021, H2HC granted more than $1 million to support innovative community-based models and critical research in New England.
H2HC is no longer awarding grants. In 2023, H2HC launched its Prizes for Innovation.
Funded Work
H2HC funding supported ground-breaking research and programs that strengthened food insecurity screening and referrals for food assistance; improved access to culturally relevant foods; and advanced partnerships between emergency food services, healthcare, and housing organizations; all through a focus on the social drivers of health.
