Health Department Assessment & Roadmap

ChangeLab Solutions, in partnership with the Centers for Disease Control and Prevention, developed the Health Department Assessment & Roadmap to help health departments evaluate their readiness to address social determinants of health through legal and policy strategies. This tool provides both an assessment of an organization’s capacity to use law and policy to tackle the root causes of health inequities and a roadmap of actionable steps for improvement. Together, the assessment and roadmap support a structured approach to building organizational capacity and advancing health equity.
